Delhi Community COPD Program
The Delhi Community COPD Program is designed for people with a diagnosis of Chronic Obstructive Pulmonary Disease. This comprehensive program aims to improve the overall health and management of those who suffer with this condition through exercise and education.
Who is eligible? People with a diagnosis of COPD or patients at risk of a “lung attack”
Who is NOT eligible? People who are medically unstable and people with end stage COPD
Key features of the program include:
- Tailored intervention to assist with confidence and capacity of managing COPD at home and in the community
- 2 hours weekly of monitored exercise and 1 hour monthly of education presented by a Certified Respiratory Educator (CRE)
- Program elements include: education (smoking cessation, medications, and nutrition), exercise, assessments (oximetry), and mental health, wellness, and peer support
Location: Delhi United Church at 105 Church St W, Delhi, ON N4B 2M1
Time: Exercise- Every Tuesday at 2:30 p.m.- 3:30 p.m. and Wednesday at 3:30 p.m.- 4:30 p.m. Education- First Wednesday of each month at 2:30 p.m. followed by the exercise
There will be three open houses on Tuesday, August 1, 15, & 29, 2017 at the Delhi United Church at 2:30 p.m. in order to provide more information about the program and give you the opportunity to meet the team.
This program is set to begin on Tuesday, September 5, 2017
