Our services include:
Many of our services are covered under private insurance plan, Provincial health care programs or on a fee for service basis. Respite services may be bundled with other services to create cost effective monthly plans. Caregiver Coaches and Nurse Advisors are available on an hourly or monthly contract. Support group and counselling services fees vary depending on membership plans. We provide bundled care options and monthly memberships that allow caregivers to reduce costs. In some cases, subsidies may be available. Monthly billing and third party billing options are available. Please contact us for more details.
Stonebridge Community Services provide services throughout the Tri-County area (Oxford, Elgin and Norfolk) in Southwestern Ontario. Eligibility and availability of services vary across the regions.
Client & Family Care Portal Access
All Care for the Caregiver Services include complimentary access to the Client and Family Care Portal. This online health care resource allows caregivers to stay on top of all care activities and provides easy and transparent communication for all the health care team. With the Client and Family Portal family members can:
- Monitor and schedule home visits
- Request additional service or change services as needed
- Receive real time health reports and care plan details
- Connect directly to loved ones through on line conferencing, remote health monitoring and tele- health
- Review billing, invoice and quickly resolve administration questions
